CATHOLIC CHARITIES OF THE DIOCESE OF PUEBLO SCAN Family Development Specialist in Pueblo, Colorado

Position: SCAN Family Development SpecialistDuties: Will provide services and supports within medical settings andhome visits to support the growth of parent-child social and emotionaldevelopment in families. The SCAN Specialist will administer social-emotionalhealth screening tools and will connect families to needed community services to improve wellness. Encourages openness and increased contacts between thehealthcare team and families and helps strengthen families to improve the socialdeterminants of family health and well-being. Proves service documentationand tracks service data.Intake - Interview families and develop and maintain relationships whichencourage families to talkcomfortably about their personal/family situation. Gather informationnecessary to assess family needsand determine eligibility and suitability for agency services, as well asother programs/servicesbeneficial to the client/family and provide orientation to those services.2. Assessment - Assist families to identify barriers which prevent them frommeeting their basic needsand determine potential solutions for overcoming these barriers. Help familiesto recognize and usetheir strengths in working toward their goal(s) by developing a FamilyDevelopment Plan and/orIndividual Employment Plan (IEP).3. Referral and Follow-up - Identify resources within the agency and/orcommunity capable of providingassistance to the family in meeting their needs. Inform and assist the familyin the process to apply forand receive services and make appropriate referrals.4. Provide Family Development Services - One-on-one instruction/informalcoaching to families, basedon the Family Development Plan/IEP which may include: Providing assistance to appropriate family members so they can obtainadditionaltraining/education which will lead to employment or employment which hashigher wages and/orbetter benefits. Encouraging the development of good self-esteem and teachingtechniques/methods to promotepositive self-esteem in other family members. Providing parenting education in the areas of child development,parent/child relationships,communication, guidance, and supervision, etc.5. Record Keeping/Reporting - Ensure that adequate program and participantrecords are maintained, andreports and paperwork are completed, as required. This includes factualrecords, documentation ofprogress and observations critical to case management, keeping accurateattendance records, travelrecords, monitoring reports, terminations, follow- ups, etc.6. Agency/Community Contact - Establish and maintain effective workingrelationships to assuresuccessful coordination efforts with community resources, including humanservice agencies,organizations, schools, employers, and individuals, and acceptreferrals from them: Utilize otherpotential resources to the advantage of families.7. Interpret various state and federal manuals used to implement agencyprograms.8. Carry out mandated reporter responsibilities as required by the ChildAbuse/Neglect law of the state ofColorado.9. Attending meetings and training as required.10. Perform other duties as assigned.Physical demands include carrying and lifting 20 lbs.; able to carry laptopsand curriculum kits, food, setting up and organizing group meetings,stairs and working in various settings.Requirements:Bachelor's Degree in Social work, Human or Child and Family Development,Education, or related field (preferred).Family Development Credential required or ability to obtain FDC within 12months of employment.Driver's license and insurable MVR and daily access to a vehicleOther Requirements:Background check required.Experience working in a social service related field.Advance level and knowledge of Microsoft office.Reliable vehicle, valid driver's license and proof of insurance and cleandriving recordShift: full time positionSalary: $35,000 yearly, non exemptBenefits:F401(k) MatchingDental Insurance, Employee Assistance ProgramHealth Insurance, Life Insurance, Vision InsuranceP
