CMHIFL is a 94 bed psychiatric facility located in Denver that is dedicated to meeting the needs of the community by providing mental health treatment to the adult population in the metro area. The mission of CMHIFL is to provide the highest quality mental health services to adults with complex, serious and persistent mental illness within the resources available. CMHIFL is dedicated to providing Trauma Informed Care and promoting the implementation of Trauma Informed Practices within the organization.

Description of Job

About this Work Unit:

The work unit provides 24 -hour inpatient psychiatric treatment services to adult psychiatric patients and their families by collaboratively planning, organizing, and executing psychiatric treatment interventions. The unit provides a high level of security and safety for individuals who present a significant degree of risk to themselves or others due to suicidal thinking, aggression, or grave disability. Services provided include: clinical assessment and diagnosis, psychological testing, treatment planning, medication evaluation and administration, individual, family, milieu and group therapies, counseling, and education, liaison and service coordination with Community Mental Health Centers (CMHCs) and Behavioral Health Organizations (BHOs) and other agencies, and discharge/ aftercare planning.

Summary of Position:

The purpose of the position is to provide psychiatrically based social work services to patients age 18-59 who have psychiatric problems, and/ or severe behavior problems resulting from acute and chronic psychiatric illnesses. This position completes comprehensive psychosocial assessments and reassessments; coordinates and documents treatment planning and treatment interventions of a multi-disciplinary team of mental health professionals including nurses, mental health clinicians, a psychologist, psychiatrists, and a music therapist. It performs individual, group and family therapies, provides case management services to assigned patients; and is responsible for the development and implementation of aftercare plans including liaison with agencies including Community Mental Health Centers, Mental Health Assessment and Service Agencies, and other entities that provide residential, educational, therapeutic, and supportive services in the community

Job Duties:

  • Complete a comprehensive bio-psychosocial assessment of adult patients being admitted for inpatient psychiatric treatment.

  • Interview the patient assuring that assessment data is as reliable as possible, that the patient is engaged in a supportive and educative manner, that confidentiality is maximized, and that symptoms and behaviors are thoroughly evaluated and handled in a sensitive and safe manner.

  • Collaborate with, coordinate, and direct staff in the development of a multidisciplinary treatment plan along with the patient and significant others, identifying treatment goals and objectives, treatment interventions and those responsible for providing them, the frequency of interventions, and the responsibility of the patient.

  • Document the plan and any issues related to the plan and revise/modify the plan according to the ongoing reassessment of the patient's progress and continuing need for treatment.

  • The position guides milieu staff by modeling patient interventions, recommending practice and patient "homework" assignments that may be implemented by them.

  • Develop a therapeutic alliance with the patient and family to secure their understanding and participation in the treatment process.

  • Direct and coordinate the treatment process and treatment recommendations of the treatment team.

  • Manage financial matters for assigned patients, determine their resource needs, and access appropriate systems to meet those needs.

  • Make all necessary referrals and complete or assist/direct the patient/family with necessary forms/ applications to ensure that all client's financial/resource/service needs are addressed or resolved to the extent possible within the available resources of the hospital and community.

  • Direct and coordinate passes, visits, and family contact.

  • Coordinate the development and implementation of efforts toward discharge and arranges, secures, and/or recommends necessary aftercare elements including, but not limited to residential placements and continuing treatment alternatives.

  • Maintain liaison with relevant and involved community agencies, services, and resources including the CMHC/BHO regarding aftercare plans and assures communication and collaboration with the patient and family.

Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights

Minimum Qualifications:

Current, valid licensure as a Social Worker (LSW) from the Colorado State

Board of Social Work Examiners.

Experience: One year of experience as a social worker.

There are no substitutions for position qualifications

Highly Desirable Knowledge, Skills and Abilities

  1. Knowledge of DSM-V

  2. Prior Adult Inpatient Experience

  3. Desire to work with Adults Who Have Been diagnosed with severe and persistent mental illness

  4. Experience and/or desire to work with diverse, under-served client populations.

Conditions of Employment:

  • Must be willing to complete background, including finger printing

  • Must be willing to complete drug screen

  • Requires Pre-employment physical

  • Must possess and maintain a valid Colorado Driver License

  • Special Qualifications: The Social Work/Counselor II requires the professional standards associated with and learned by being a licensed LSW and possession of licensure and one year post LSW experience.

  • May need to be able to deal with potentially aggressive patients. May need to be assigned to perform social work services on treatment teams throughout the hospital as needed to assure continuity of care and appropriate coverage related to workload demands and availability of staff on other units. May need to work evenings or weekends to meet the needs of patients, family, and institution. May need to receive calls for information or advice regarding assigned patients outside of normal working hours.
